2021 Weatherly Mobile Vaccine Clinic

Lehigh Valley Health Network is proud to partner with the Weatherly Area School District to provide a Community Covid-19 Vaccination Clinic on Tuesday, June 29th, 2021, from 4:00pm to 7:00pm. The vaccination clinic will be held at Weatherly Area Middle School: 602 Sixth Street, Weatherly, PA 18255 and is open to anyone 12 years of age and older. Please enter through the middle school gymnasium doors - signage will be posted at the appropriate entrance as well.

Please bring a completed consent for vaccination and review the Emergency Use Authorization form prior to vaccination. Anyone under the age of 18 will need to be accompanied by a parent/guardian to receive the vaccination.
